Geriatric Oncology is a new specialty within the world of cancer care and we are proud to offer this specialty of care to both our current and prospective patient population with a rigorous research and academically based approach that few health systems have the ability to provide.
Melani Shaum, MD serves as the Director of Geriatric Oncology at The Angeles Clinic and Research Institute (link to bio), she has decades of experience in the field of Oncology and has contributed greatly in the battle against cancer throughout her career both in the provision of patient care and her contributions to research on cancer.
Background
The majority of cancer patients are older than 65 in the United States. Geriatric patients are considered to be those aged 65 and above (based on the merck manual definition). While most cancers are approached in a similar fashion between the ages of 65 and 70 it is the approach in those over the age of 70 that can become complex and require special considerations with regards to treatment options. Unfortunately, in some cases patients may be under treated or not treated at all as age 70 is seen by some physicians as a risk on its own.
Age though is not the only deciding factor in deciding what the best individualized care plan is for you or a loved one. Often patients older than 70 have underlying health conditions, perhaps one or even several that could serve as determining factors in whether to pursue one treatment option over another. There may also be challenges for those over 70 such as the inability to drive, difficulty grocery shopping or preparing healthy food for meals. Social Determinants impact individual health even more so when there are physical health conditions present.
What to expect during the appointment
Patients whether current or those who are just seeking a second opinion can expect a unique approach while under our care. First time appointments are not your typical half hour exam. Initial consultations are comprehensive and patients would meet with a Geriatric Oncologist for one hour and receive a Comprehensive Geriatric Assessment where screenings are performed for: Frailty, Weight Loss, Nutrition, Autonomy, Depression and Cognition. The Vulnerable Elders Survey and French G8 exam will also be performed.
Following the physician visit patients then spend a second hour with a Geriatric Specialist who could help assist with the plan of care in determining additional health and social needs in addition to the assessment performed by the physician.
